Full payment is due upon registration. Cancelations with less than 30 days notice are non-refundable. If The Social Club cancels an outing with reasons beyond its control a full refund will be granted. PAYMENTS CAN BE DONE THROUGH E-TRANFER TO:

DISCLOSURE

The individual and their representative agrees that they have disclosed to The Social Club every relevant matter to their condition which might impact upon our ability to provide the participant with the appropriate level of care while participating in our service. Failure to disclose relevant information regarding behaviour that is likely to affect the smooth operation of the activity; adversely affect the enjoyment of others, or the safety of themselves and others may result in removal at any time in any program without reimbursement.

CONSENT TO MEDICAL TREATMENT

You also give permission to The Social Club to administer prescription medications as perscribed and required.

LIABILITY

While every precaution is taken for the safety and health of our clients; By submitting this form you acknowledge you are releasing The Social Club from any and all actions, cause of action, claims and demands resulting from any loss, injury or damage to person or property. Not withstanding that any such loss, unjury or damage may have arisen by reason of neglience of the releases.
